Health and Medicine

FSI’s researchers assess health and medicine through the lenses of economics, nutrition and politics. They’re studying and influencing public health policies of local and national governments and the roles that corporations and nongovernmental organizations play in providing health care around the world. Scholars look at how governance affects citizens’ health, how children’s health care access affects the aging process and how to improve children’s health in Guatemala and rural China. They want to know what it will take for people to cook more safely and breathe more easily in developing countries.

FSI professors investigate how lifestyles affect health. What good does gardening do for older Americans? What are the benefits of eating organic food or growing genetically modified rice in China? They study cost-effectiveness by examining programs like those aimed at preventing the spread of tuberculosis in Russian prisons. Policies that impact obesity and undernutrition are examined; as are the public health implications of limiting salt in processed foods and the role of smoking among men who work in Chinese factories. FSI health research looks at sweeping domestic policies like the Affordable Care Act and the role of foreign aid in affecting the price of HIV drugs in Africa.

Daniel Posner is Total Professor of Political Science at the Massachusetts Institute of Technology, and currently a fellow at the Center for Advanced Study in the Behavioral Sciences.  His research focuses on ethnic politics, research design, distributive politics and the political economy of development in Africa. His work investigates, among other topics, the sources of ethnic identification and the political, social and economic outcomes that ethnicity affects-coalition-building, voting, collective action, public goods provision, and economic growth-with special attention to the mechanisms through which it has its impact. His methodological approach is to find creative ways to maximize leverage for making strong descriptive and causal claims, through the use of experiments (in the lab, in the field, and occurring "naturally"), new data sources (including the re-appropriation of data collected for other purposes), and the adoption of techniques from other disciplines such as satellite geography, public health, and behavioral economics.

His most recent co-authored book, Coethnicity: Diversity and the Dilemmas of Collective Action (Russell Sage, 2009) employs experimental games to probe the sources of poor public goods provision in ethnically diverse communities. His first book, Institutions and Ethnic Politics in Africa (Cambridge, 2005), explains why and when politics revolves around one dimension of ethnic cleavage rather than another. He has received several awards for his work, including the Luebbert Award for best book in Comparative Politics (2006 and 2010), the Heinz Eulau Award for the best article in the American Political Science Review (2008), the Michael Wallerstein Award for the best article in Political Economy (2008), the best book award from the African Politics Conference Group (2006), and the Sage Award for the best paper in Comparative Politics presented at the APSA annual meeting (2004). He has been a Harvard Academy Scholar (1995-98), a National Fellow at the Hoover Institution (2001-02), a Carnegie Scholar (2003-05) and, this year, a Fellow at the Center for Advanced Study in the Behavioral Sciences (2010-11). He currently serves on the editorial boards of World Politics, PS, and the Annual Review of Political Science. He is the co-founder of the Working Group in African Political Economy (WGAPE). He received his BA from Dartmouth College and his PhD from Harvard University. Before moving to MIT, he taught for twelve years at UCLA.

It is now widely recognized that a rigorous, policy-relevant impact evaluation embeds the counterfactual analysis of impact in a wider analysis of the underlying program theory (theory of change) of the intervention, also referred to as causal chain analysis. Unpacking the causal chain requires a combination of factual and counterfactual analysis. The seminar will present examples of causal chains. The types of data collection and analysis – both quantitative and qualitative – to analyze the different links in the chain will be discussed. A major challenge in mixed methods is to truly integrate quantitative and qualitative approaches.

About the speaker

Howard White formerly led the impact evaluation program of the Independent Evaluation Group of the World Bank, where he was responsible for impact studies on basic education in Ghana, health and nutrition in Bangladesh, rural electrification, rural development in Andhra Pradesh and a review of impact studies of water supply and sanitation.

The Stanford Program on International and Cross-Cultural Education (SPICE) presented two workshops at the 2011 EARCOS Teachers' Conference in Kota Kinabalu, Malaysia.

On March 24, 2011, SPICE conducted a workshop entitled "The Atomic Bombings and Their Legacies." This session introduced upper elementary and secondary school teachers to activities and resources from the SPICE curriculum units "Examining Long-Term Radiation Effects" and "Sadako's Paper Cranes and Lessons of Peace." Given the recent events in Japan, SPICE focused on presenting content from the curriculum unit, "Examining Long-Term Radiation Effects," and worked with participants to develop classroom activities to engage their students in a discussion about nuclear issues.

On March 25, 2011, SPICE presented a second workshop entitled "Infectious Diseases and Global Public Health." This session introduced secondary school teachers to lessons and activities from two SPICE curriculum units: "Infectious Diseases and Global Public Health" and "TeachAIDS: A Comprehensive HIV/AIDS Prevention Curriculum."  Participants engaged in a series of interactive activities and learned about new online teacher resources from SPICE and TeachAIDS, http://teachaids.org/.

The East Asia Regional Council of Schools (EARCOS) is an organization of 120 member schools in East Asia. EARCOS' mission is to inspire adult and student learning through its leadership and service.

The February 10 Liberation Technology seminar titled, Can ICT Improve Clean Water Delivery Systems in Slums? Lessons from Kibera was led by two Stanford students, Katherine Hoffman, M.A. Candidate in International Policy Studies and Global Health together with Sunny Jeon, PhD candidate in Political Science. Hoffman and Jeon presented on the topic of the M-Maji system, a start-up non-profit project that uses mobile phones to empower communities with better information about water availability, price, and quality. M-Maji emerged from the Designing Liberation Technologies course taught at the Stanford d.school, which is dedicated to using mobile phone technology for health improvement in Kibera.  

The M-Maji project was conceived to confront a specific need in one of Africa's largest informal settlements: Nairobi's Kibera slum. Kibera holds about one quarter of Nairobi's population, but lacks a formal water and sanitation system. In short, water in Kibera is scarce, costly, uncertain, and often contaminated. The cost of water can rise up to over $3 per square meter during a shortage, and often takes up 20% of residents' income. Water is uncertain because about 40% of vendors in Kibera do not have access to a constant supply of water, especially since many of the connections that they do use are illegal. Additionally, water is often contaminated because water pipes run through sewer areas and are often plastic, since metal pipes would be taken as scrap. The result of these conditions is that the average person spends about 55 minutes per day looking for water, and about 68% of residents use informal water kiosks to access water.

M-Maji is a mobile platform designed to address problems facing both the seller and buyer of water in Kibera. Sellers report information about water availability and price at their water kiosk, enabling them to attract customers. Meanwhile, buyers access the system to find information about where water is available, at what price and at what level of quality. A third feature of the platform enables complaints and feedback regarding water sources. By coordinating and centralizing water information from multiple sources, M-Maji is designed to empower residents with better information about water availability, price, and quality, which ultimately helps to improve access to clean water.

There are several key reasons why a mobile platform that tackles the information side of the problem may be effective in solving the water problem in Kibera:

  1. The water market is distorted, but not coordinated. Also, there is significant variation between neighborhoods.
  2. Information asymmetry exists that is not solved by other sources or tools.
  3. M-maji is needs-driven.
  4. Relative to other interventions, M-Maji is cost-sensitive and low tech.

Although fieldwork conducted by the M-Maji team has indicated great potential for the platform's success, the team has also faced many challenges in implementation. One example stems from the platform's use of USSD sessions. Like SMS, these sessions allow short messages (up to 182 characters) to be exchanged. Users must enter a short code to begin a session, which then lasts about 2 minutes. However, the team has faced significant difficulty in getting mobile operators to allow access to USSD (in Swahili), as well as to negotiate rate agreements with mobile operators. The involvement of gangs and government in the water market introduces another challenges. In some cases, gangs bribe utility people to restrict the supply to push up prices, which may hinder the eventual effectiveness of the M-Maji tool.

Following the formation of a partnership with an important local NGO in Kibera called Umande Trust, M-Maji is moving rapidly into the pilot stage. The team is currently working to carry out a randomized impact evaluation to obtain convincing estimates of M-Maji's cost-effectiveness and impact on water outcomes. With the help of the community partner, the team is running a six-month long pilot on the West side of Kibera. In addition to surveying 1000 households for the baseline survey, the team is also carrying out data collection on water prices and water quality. M-Maji will officially launch in May or June of 2011, and the team will continue to reassess the project to better meet user needs as the results of the pilot become available.

Alexis Medina is the Associate Director for Research Programs at SCCEI, and also Associate Director of the Rural Education Action Program (REAP). At SCCEI, she helps to connect China-oriented faculty and graduate students with university resources and with one another, as well as to navigate administrative bureaucracies on both sides of the Pacific. She has been directly involved in building research networks and fostering connections between the US and China since 2006, and has extensive experience in international program management, including leading survey teams in rural China, overseeing the design and development of field projects, and coordinating data collection efforts and analysis. Her academic expertise lies at the intersection of health and education, and she has co-authored dozens of academic publications in this area. Alexis speaks fluent Mandarin, and has previously held research positions at the Harvard T.H. Chan School of Public Health, and Shandong University in China.

How can Stanford University contribute to improved health policy in the low-income countries of Asia? In addition to our formal degree-granting and research programs, mentorship of young scholars can play a role in strengthening the evidence base and analytic skills undergirding health policy for the millions of Asians who live on only a few dollars a day.

The Asia Health Policy Program (AHPP) is delighted to announce the inauguration of a fellowship program for young health policy experts from developing countries in Asia. The first Developing Asia Health Policy Fellow will be Dr. Siyan Yi from Cambodia. He holds a medical doctor degree from the University of Health Sciences in Cambodia as well as a doctoral degree in international health sciences from the Graduate School of Medicine at the University of Tokyo. Dr. Karen Eggleston, director of AHPP, will be his primary sponsoring faculty member.

Dr. Yi's research has centered largely on epidemiological methods. This has included, for example, work on surveys in Cambodia on adolescent risky sexual behaviors, substance abuse, and depression; a health promotion project in primary schools; sexual behaviors among people living with HIV/AIDS; and HIV risk behaviors among tuberculosis patients. Currently, he is involved in hospital- and community-based research projects in several developing countries as well as in Japan.

Cambodia has recently created a School of Public Health and is facing an increasing burden of chronic disease, while the burden from infectious diseases such as tuberculosis remains significant. Since Dr. Yi's research interests lie in health promotion and risky behaviors, he could someday be instrumental in helping to set policies to address the public health challenges Cambodia will face.  

Dr. Yi will join the Walter H. Shorenstein Asia-Pacific Research Center (Shorenstein APARC) in autumn 2011 for a nine-month fellowship to immerse in the academic environment and to develop additional skills to contribute to improved health policy in Cambodia. He notes that "the breadth of [Shorenstein] APARC's work is truly impressive... I look forward to making full use of the resources and opportunities afforded me through the visiting fellowship."

The new health reform started in 2009 has shown the determination of the Chinese government, especially the central government, to increase its responsibility in the health sector. The most obvious manifestation of this commitment would be to increase government health expenditure (GHE). But there is still a hot debate about whether the government should allocate more public finds to health or just deepen the marketization of the health sector. Moreover, commitments at the central and local levels are not the same: local government responsibility for GHE is high, and commitments by the central government to increase GHE have not translated into increases in local government GHE as much as proposed in the national health reform.

Our research seeks to answer two questions: What was the actual pattern of GHE? And why did China’s local governments respond as they did? We first discuss the necessity of public financing for health care, and then analyze how intergovernmental economic competition affects local governments’ behavior under “Chinese-style decentralization” (known as fiscal decentralization with political centralization). Empirically, we apply a dynamic panel data model to provincial panel data from 1991 to 2007 to identify the effect of GHE on health performance in each province over time, using infant mortality and some morbidity metrics as health performance variables. We also examine differences across regions, as well as before and after the Severe Acute Respiratory Syndrome (SARS) epidemic of 2003.

Our analysis provides evidence that Chinese-style decentralization negatively impacted GHE. The main findings are as follows:

  1. Increasing GHE did improve health performance, and this improvement was mainly driven by the GHE through the health department directly, not through spending by other governmental departments that also impact health. However, pursuit of economic performance lowered local governments’ GHE, mainly by decreasing GHE through local health departments.
  2. Compared with in the eastern and western regions, this health improvement was not significant in China’s middle regions, where the intergovernmental economic competition leads to much less GHE through health departments.
  3. The outburst of SARS in 2003 further increased the positive effect from GHE through local health departments, while the effect from GHE through other departments was not equally significant.

All these results suggest that adjusting the structure of public health financing, reforming the fiscal system, and improving the performance evaluation system for local governments are critical for the success of China’s on-going health reform.

2011 Shorenstein-Spolgi Fellow in Comparative Health Policy

Qiulin Chen is a postdoctoral fellow of Shorenstein APARC and a member of the center's Asia Health Policy Program. His main interest of research is health economics and public finance, focusing on policy and outcome comparison of health care systems and Chinese health reform. His dissertation focused on performance comparison between public (or governmental) and private health care financing, between local and central government responsibility on health care, between contracted and integrated health care system. In particular, his dissertation examined under Chinese-style decentralization, known as fiscal decentralization with political centralization, how economic competition affect local government's behaviour on health investment, and why public contracted system obstructs health performance and provides one channel of such effects in terms of preventive care and public health. He is currently involved in a comparative research project on demographic change in East Asia based on the National Transfer Accounts data and analysis.

Chen's recent publication is "The changing pattern of China's public services" (with Ling Li and Yu Jiang) in Population Aging and the Generational Economy: A Global Perspective (Ronald Lee and Andrew Mason, editors), forthcoming 2011. Before studying in Stanford, he has published more than 10 papers in academic journals in Chinese, such as Jing Ji Yan Jiu (Economic Research) and Zhong Guo Wei Sheng Jing Ji (Chinese Health Economics), and 5 book chapters. He has participated in about 20 research projects, such as A Design of Framework for Healthcare Reform in China which is commissioned by the State Council Working Party on Health Reform, Strategy Planning Study of "Healthy China 2020" which is commissioned by the Minister of Health, and Health Challenge in the Aging Society and It's Policy Implication funded by Chinese National Natural Science Foundation.

Chen earned his Ph.D. in Economics from Peking University in 2010, and earned a B.A. in Business Administration from Nanjing University in 2001. From 2004 through 2008, he was Executive Assistant of the Director of the China Centre for Economic Research at Peking University (CCER). He is also a postdoctoral fellow of National School of Development at Peking University (Its predecessor is CCER).

Pradnya Palande's passion for improving human health through scientific research began during her graduate studies, and has continued to the present day through her work with Reliance Life Sciences (RLS) of India. Palande, a 2010-2011 Corporate Affiliates Fellow, holds a master's degree in zoology, with a specialization in animal physiology and a minor in biotechnology. "I used to dream of getting a job in the biotechnology industry," she says. Within just a few short months of graduating, Palande began working for RLS, and she has enjoyed serving the company for the past nine years.

RLS belongs to the Reliance Group, one of India's largest corporate entities. It conducts research in a wide variety of areas, such as stem cell therapy, molecular medicine, and industrial biotechnology, and it offers products ranging from therapeutic proteins to tissue-cultured plants. "RLS is a life sciences company in the truest sense," Palande states. In her role as a team leader for the company's biopharmaceuticals division, which is headed by Dr. Venkata Ramana, she conducts literature searches and strategic planning for new research projects, and also supports regulatory audits. In addition to her primary duties, Palande is dedicated to her role as secretary of the safety observation committee. "RLS believes in safety and a safe working culture," Palande emphasizes. "It is a perfect place for me to work."

During her year at the Walter H. Shorenstein Asia-Pacific Research Center (Shorenstein APARC), Palande is conducting cancer-related research in a laboratory under the direction of Cliff L. Wang of the Department of Chemical Engineering. Her Shorenstein APARC advisors are Rafiq Dossani, a senior research scholar, and Karen Eggleston, the director of the Asia Health Policy Program. Palande and her fellow researchers in Wang's lab are studying the population dynamics of cancer cells. In the process of cancer development, Palande explains, it is thought that cells often develop high mutational activity before accumulating mutations that lead to cancer. Mutations—changes in the genome sequence—can be advantageous, benign, or harmful to cells. Palande is examining whether a high mutation rate provides a survival advantage to cancer cells. For this, she is studying the activation-induced cytidine deaminase (AID) enzyme, which plays a key role in the development of immune response in the human body. The goal of the project is to provide insight into the nature of cancer development, and also to give direction for the prevention of chemotherapy resistance due to AID activity.

"Before I leave for India, I would like to complete my project successfully in a way that will instigate future research in cancer biology," Palande says. Upon returning to RLS, she intends to apply some of the concepts that she has gained while conducting research at Stanford. She also hopes to build upon the Stanford management classes that she has been auditing, studying the subject in more detail with the intention of utilizing it in her work at RLS.

"It has been a great privilege for me to work at Stanford, and it has been an amazing experience," she states. "Interacting with people from various backgrounds has been enlightening. Also, I am quite impressed by the professionalism of people [in the United States], their helping nature, and their free way of life. I love it." She adds that through her Stanford research and classes, her horizons have been broadened.

As passionate about the natural world as she is about scientific research, Palande looks forward to traveling more before returning to India. "I love traveling to be close to nature," she says. Also something of an adventurer, she has taken a rock-climbing course and has tried skydiving. Inspired by the range of activities in the United States, she plans to take an activity class, such as dance, after returning to India. "I want to keep myself as active and healthy as possible," she says with a smile.

In partnership with the Center for Health Policy (CHPPCOR) at Stanford, this research initiative brings together medical doctors, health economists, and political scientists seeking to understand infant mortality declines in the post-War Era. The research initiative develops new measures of political incentives for population health improvement embedded in finely grained political institutions.

Paul Wise is a clinical professor of pediatrics and a CHP/PCOR core faculty member. His work focuses on children's health policy; health disparities by race, ethnicity and socioeconomic status; and the interaction of genetics and the environment as these factors influence child and maternal health.

Before coming to Stanford in July 2004, he was a professor of pediatrics at Boston University and vice-chief of Social Medicine and Health Inequalities at Brigham and Women's Hospital. He previously served as director of emergency and primary care services at the Children's Hospital of Boston, and as director of the Harvard Institute for Reproductive and Child Health at Harvard Medical School. He has also served as a special expert at the National Institutes of Health and as special assistant to the U.S. Surgeon General.

Wise has worked to improve healthcare practices and policies in developing countries. He is involved in child health projects in India, South Africa and Latin America, targeting diseases such as tuberculosis and AIDS. He currently chairs the steering committee of the NIH's Global Network for Maternal and Child Health Research, and he has served on many other boards and committees including the Physicians' Task Force on Hunger and the American Academy of Pediatrics' Consortium on Health Disparities. He has received honors from organizations including the American Public Health Association, the March of Dimes, and the New York Academy of Medicine.

He received a BA in Latin American studies from Cornell University, an MD from Cornell University and an MPH from the Harvard School of Public Health. He completed a residency in pediatrics at Children's Hospital Medical Center in Boston.

Dr. Paul Wise is dedicated to bridging the fields of child health equity, public policy, and international security studies. He is the Richard E. Behrman Professor of Child Health and Society and Professor of Pediatrics, Division of Neonatology and Developmental Medicine, and Health Policy at Stanford University. He is also co-Director, Stanford Center for Prematurity Research and a Senior Fellow in the Center on Democracy, Development, and the Rule of Law, and the Center for International Security and Cooperation, Freeman Spogli Institute for International Studies, Stanford University. Wise is a fellow of the American Academy of Arts and Sciences and has been working as the Juvenile Care Monitor for the U.S. Federal Court overseeing the treatment of migrant children in U.S. border detention facilities.

Wise received his A.B. degree summa cum laude in Latin American Studies and his M.D. degree from Cornell University, a Master of Public Health degree from the Harvard School of Public Health and did his pediatric training at the Children’s Hospital in Boston. His former positions include Director of Emergency and Primary Care Services at Boston Children’s Hospital, Director of the Harvard Institute for Reproductive and Child Health, Vice-Chief of the Division of Social Medicine and Health Inequalities at the Brigham and Women’s Hospital and Harvard Medical School and was the founding Director or the Center for Policy, Outcomes and Prevention, Stanford University School of Medicine. He has served in a variety of professional and consultative roles, including Special Assistant to the U.S. Surgeon General, Chair of the Steering Committee of the NIH Global Network for Women’s and Children’s Health Research, Chair of the Strategic Planning Task Force of the Secretary’s Committee on Genetics, Health and Society, a member of the Advisory Council of the National Institute of Child Health and Human Development, NIH, and the Health and Human Secretary’s Advisory Committee on Infant and Maternal Mortality.

Wise’s most recent U.S.-focused work has addressed disparities in birth outcomes, regionalized specialty care for children, and Medicaid. His international work has focused on women’s and child health in violent and politically complex environments, including Ukraine, Gaza, Central America, Venezuela, and children in detention on the U.S.-Mexico border.
