Philanthropist and software giant Bill Gates spoke to a Stanford audience last week about the importance of foreign aid and product innovation in the fight against chronic hunger, poverty and disease in the developing world.

His message goes hand-in-hand with the ongoing work of researchers at Stanford’s Freeman Spogli Institute for International Studies. Much of that work is supported by FSI’s Global Underdevelopment Action Fund, which provides seed grants to help faculty members design research experiments and conduct fieldwork in some of the world’s poorest places.

Four FSI senior fellows – Larry Diamond, Jeremy Weinstein, Paul Wise and Walter Falcon – respond to some of the points made by Gates and share insight into their own research and ideas about how to advance and secure the most fragile nations.

Without first improving people’s health, Gates says it’s harder to build good governance and reliable infrastructure in a developing country. Is that the best way to prioritize when thinking about foreign aid?

Larry Diamond: I have immense admiration for what Bill Gates is doing to reduce childhood and maternal fatality and improve the quality of life in poor countries.  He is literally saving millions of lives.  But in two respects (at least), it's misguided to think that public health should come "before" improvements in governance.  

First, there is no reason why we need to choose, or why the two types of interventions should be in conflict.  People need vaccines against endemic and preventable diseases – and they need institutional reforms to strengthen societal resistance to corruption, a sociopolitical disease that drains society of the energy and resources to fight poverty, ignorance, and disease.  

Second, good governance is a vital facilitator of improved public health.  When corruption is controlled, public resources are used efficiently and justly to build modern sanitation and transportation systems, and to train and operate modern health care systems.  With good, accountable governance, public health and life expectancy improve much more dramatically.  When corruption is endemic, life-saving vaccines, drugs, and treatments too often fall beyond the reach of poor people who cannot make under-the-table payments. 

Foreign aid has come under criticism for not being effective, and most countries have very small foreign aid budgets. How do you make the case that foreign aid is a worthy investment?

Jeremy M. Weinstein: While foreign aid may be a small part of most countries’ national budgets, global development assistance has increased markedly in the past 50 years. Between 2000 and 2010, global aid increased from $78 billion to nearly $130 billion – and the U.S. continues to be the world’s leading donor.

The challenge in the next decade will be to sustain high aid volumes given the economic challenges that now confront developed countries. I am confident that we can and will sustain these volumes for three reasons.

First, a strong core of leading voices in both parties recognizes that promoting development serves our national interest. In this interconnected world, our security and prosperity depend in important ways on the security and prosperity of those who live beyond our borders.

Second, providing assistance is a reflection of our values – it is these humanitarian motives that drove the unprecedented U.S. commitment to fighting HIV/AIDS during the Bush Administration.

Perhaps most importantly, especially in tight budget times, development agencies are learning a great deal about what works in foreign assistance, and are putting taxpayers’ dollars to better use to reduce poverty, fight disease, increase productivity, and strengthen governance – with increasing evidence to show for it.

Some of the most dire situations in the developing world are found in conflict zones. How can philanthropists and nongovernmental organizations best work in places with unstable governments and public health crises? Is there a role for larger groups like the Gates Foundation to play in war-torn areas?

Paul H. Wise: As a pediatrician, the central challenge is this: The majority of preventable child deaths in Sub-Saharan Africa and in much of the world occur in areas of political instability and poor governance. 

This means that if we are to make real progress in improving child health we must be able to enhance the provision of critical, highly efficacious health interventions in areas that are characterized by complex political environments – often where corruption, civil conflict, and poor public management are the rule. 

Currently, most of the major global health funders tend to avoid working in such areas, as they would rather invest their efforts and resources in supportive, well-functioning locations.  This is understandable. However, given where the preventable deaths are occurring, it is not acceptable. 

Our efforts are directed at creating new strategies capable of bringing essential services to unstable regions of the world.  This will require new collaborations between health professionals, global security experts, political scientists, and management specialists in order to craft integrated child health strategies that respect both the technical requirements of critical health services and the political and management innovations that will ensure that these life-saving interventions reach all children in need.

Gates says innovation is essential to improving agricultural production for small farmers in the poorest places. What is the most-needed invention or idea that needs to be put into place to fight global hunger?

Walter P. Falcon: No single innovation will end hunger, but widespread use of cell phone technology could help.

Most poor agricultural communities receive few benefits from agricultural extension services, many of which were decimated during earlier periods of structural reform. But small farmers often have cell phones or live in villages where phones are present.

My priority innovation is for a  $10 smart phone, to be complemented with a series of very specific applications designed for transferring knowledge about new agricultural technologies to particular regions.  Using the wiki-like potential of these applications, it would also be possible for farmers from different villages to teach each other, share critical local knowledge, and also interact with crop and livestock specialists.

Language and visual qualities of the applications would be key, and literacy problems would be constraining.  But the potential payoff seems enormous.

On May 18-19, the Program on Poverty and Governance will host a two-day conference on the provision of public goods and good governance throughout the world. This conference, co-sponsored by the Center for Latin American Studies, will bring together an interdisciplinary group of economists, political scientists, policymakers, and public health researchers examining these questions. Public goods—goods and services such as education, healthcare, sanitation, potable water, and other benefits provided by the government—are intrinsically tied to issues of governance, which we broadly define as the exercise of political authority and the use of institutional resources to manage society’s problems and affairs. More specifically, factors such how political leaders get elected, the way in which government projects get funded, whether the community participates in decision-making, and the extent to which the distribution of government benefits is done through clientelistic networks, among others, play an important role in the quality and coverage of public goods that governments provide. Additionally, a critical question in large parts of the developing world relates to the role of international players and foreign aid in the provision of public goods –does external provision of public goods enable or hinder governmental capacity to deliver public services in poor communities? Through two days of presentations and panel discussions, the conference will explore how various facets of governance affect the provision of public goods and services throughout the world.

Beatriz Magaloni Magaloni is the Graham Stuart Professor of International Relations at the Department of Political Science. Magaloni is also a Senior Fellow at the Freeman Spogli Institute, where she holds affiliations with the Center on Democracy, Development and the Rule of Law (CDDRL) and the Center for International Security and Cooperation (CISAC). She is also a Stanford’s King Center for Global Development faculty affiliate. Magaloni has taught at Stanford University for over two decades.

She leads the Poverty, Violence, and Governance Lab (Povgov). Founded by Magaloni in 2010, Povgov is one of Stanford University’s leading impact-driven knowledge production laboratories in the social sciences. Under her leadership, Povgov has innovated and advanced a host of cutting-edge research agendas to reduce violence and poverty and promote peace, security, and human rights.

Magaloni’s work has contributed to the study of authoritarian politics, poverty alleviation, indigenous governance, and, more recently, violence, crime, security institutions, and human rights. Her first book, Voting for Autocracy: Hegemonic Party Survival and its Demise in Mexico (Cambridge University Press, 2006) is widely recognized as a seminal study in the field of comparative politics. It received the 2007 Leon Epstein Award for the Best Book published in the previous two years in the area of political parties and organizations, as well as the Best Book Award from the American Political Science Association’s Comparative Democratization Section. Her second book The Politics of Poverty Relief: Strategies of Vote Buying and Social Policies in Mexico (with Alberto Diaz-Cayeros and Federico Estevez) (Cambridge University Press, 2016) explores how politics shapes poverty alleviation.

Magaloni’s work was published in leading journals, including the American Political Science Review, American Journal of Political Science, Criminology & Public Policy, World Development, Comparative Political Studies, Annual Review of Political Science, Cambridge Journal of Evidence-Based Policing, Latin American Research Review, and others.

Magaloni received wide international acclaim for identifying innovative solutions for salient societal problems through impact-driven research. In 2023, she was named winner of the world-renowned Stockholm Prize in Criminology, considered an equivalent of the Nobel Prize in the field of criminology. The award recognized her extensive research on crime, policing, and human rights in Mexico and Brazil. Magaloni’s research production in this area was also recognized by the American Political Science Association, which named her recipient of the 2021 Heinz I. Eulau Award for the best article published in the American Political Science Review, the leading journal in the discipline.

She received her Ph.D. in political science from Duke University and holds a law degree from the Instituto Tecnológico Autónomo de México.

FSE director Roz Naylor participated in the lead plenary session integrating climate, energy, food, water, and health at the 12th National Conference on Science, Policy and the Environment. The theme of this year's conference was Environment and Security, and included keynote talks delivered by Amory Lovins and Thomas Freedman.

While many of us here in the US wake up concerned about political, economic, and military unrest at home and abroad, billions still wake up with more basic, human security concerns, opened FSE director Rosamond L. Naylor in a plenary connecting climate, energy, food, water, and health.

Are we going to have enough to eat today? How am I going to feed my family or care for family members struggling with HIV/AIDS and other infectious diesease? Is there enough water to drink, bathe, and still water my crops?

Naylor emphasized the need to bring these human security issues back into the forefront of our global conscious. While these are 'humanitarian needs at the core', they are also related to national and international security.

"When people are desperate enough, and we've seen this particularly with the food price spike in recent years, they take to the streets, and sometimes when they take to the streets they realize they are disgruntled about a number of things in addition to food prices," said Naylor.

The Arab Spring and wave of rebellions throughout the Middle East last year demonstrate the connections between food security, unmet basic needs, and national security. It has been a chaotic time for world food markets, said Naylor.

Naylor's global statistics are discouraging. Over a billion people still suffer from chronic hunger and malnutrition, 1/5 don't have physical access to water, and roughly 1.6 billion are facing economic water constraints (do not have the economic resources to access available water). Food and water insecurity are exacerbating the incidence and transmission of infectious disease.

At a time when investment is sorely needed, the Hill has been making dramatic cutbacks in foreign assistance and foreign investment is falling short. Efforts made by the private sector, philanthropy, and civil society, while valuable, remain siloed. Opportunties are being missed by not addressing the interrelated nature of food and health issues.

Despite this dire outlook, Naylor offered solutions to help us rethink our development strategy.

  1. Invest in more diversified and nutritious crops that have more climate adaptation potential.
  2. Consider new irrigation strategies, particularly in areas like Africa where 96% of the continent is still not irrigated. Not large dams, but small, distributed irrigation systems that rely on solar and wind.
  3. Integrate food and health programs and the way we think about domestic and productive water uses.

Naylor was joined on the panel by Jeff Seabright (Vice President, The Coca-Cola Company), Daniel Gerstein (Deputy Under Secretary for Science and Technology, U.S. Department of Homeland Security), and Geoff Dabelko (Woodrow Wilson International Center for Scholars). About the topic: A major challenge faced by President Obama is how to modernize the system of global governance to adapt to the rising influence of emerging powers and to more effectively address new, cross-cutting challenges. The Obama Administration has pursued a variety of strategies in this respect from the reform of existing institutions to the creation of new multilateral processes and mechanisms. How effectively these efforts are working - and to what extent these institutions actually change the behavior of states - remains an open question. Drawing on his experiences at the National Security Council, Weinstein will discuss the Obama Administration's approach to global governance, and in particular the efforts of the Administration to shape a more effective anti-corruption regime internationally, through the G-20 and the creation of the Open Government Partnership.

Abstract
Weinstein recently returned from two years on the National Security Council staff at the White House where we played a key role in thinking through a 21st century approach to fighting corruption and strengthening governance. One important initiative was President Obama’s Open Government Partnership (OGP), a multilateral effort involving more than 50 countries, hundreds of civil society groups, and leading technology companies, with the goal of making governments more transparent, more accountable, and more effective. Weinstein will offer a behind-the-scenes perspective on the creation of OGP, and discuss its promise and potential pitfalls.

Karl Eikenberry has a unique perspective on the U.S. war in Afghanistan. The former ambassador to Kabul, his 35-year career in the army includes an 18-month tour as commander of the U.S.-led coalition forces in the country. As the conflict hit the 10-year mark, Eikenberry discussed President Obama’s Afghanistan strategy, the challenge of working with Pakistan, and the problem of overpromising.

We are coming up on 10 years in Afghanistan. What’s your snapshot of where we are now and where Afghanistan is going?

We're on the right path. The president has adopted a strategy that by the end of 2014 – if it’s well implemented – will have us in a position, and have the Afghans in a position, where the Afghans will be fully responsible for providing their own security.

It's going to require that the Afghan army and the Afghan police continue to develop sufficient capabilities so that by 2014 they have the right numbers and the right quality to allow our military forces to step back into a supporting role. It's going to require that the Afghan government continues to make improvements in terms of its accountability and its responsiveness to its people. And third, it's going to require that Pakistan make more efforts to attack the sanctuaries that Afghan Taliban currently enjoys there.

That doesn't mean we’ll be at a point where the U.S. commitment in Afghanistan ends. We'll continue to provide security assistance to the Afghan national security forces beyond 2014. We will continue to have a robust diplomatic mission at the end of 2014. I would expect that we'll still have a substantial foreign assistance program to Afghanistan – not at the level it is right now, but still substantial by global standards, and we’ll still, I expect, remain very diplomatically engaged in that part of the world.

So you've laid out three things: capacity building, governance, and Pakistan. Can we accomplish all three?

I think we have a reasonable possibility for success. I would not put probability against that. We know how to do capacity building especially with security forces, and I'd say over the last decade, we’ve made some important gains in knowing how to do that. It takes time, it takes resources, and it takes patience. The second thing – "good governance" – that's harder.

Ultimately, you can only have success in the first category of capacity building if you have success in the second category because all those institutions have to rest upon a foundation of what the people might say is reasonable, good governance, something that they’re willing to voluntarily support. That's more problematic.

The third category, Pakistan, is even more problematic. Their support of the Afghan Taliban is still seen by some elements within the state of Pakistan as being in their national security interest.

Are there things you think the U.S. policy makers have learned in Afghanistan that can be applied elsewhere?

I do. If you look in any of the domains we’re working in in Afghanistan – security assistance, rule of law, education and health – there are good lessons we've learned over time. Americans are extraordinarily adaptive. We’re creative. One of our good characteristics is that we frequently pull back from an enterprise, sum up lessons learned, be self critical, and continue to improve.

Another lesson learned coming out of Afghanistan may be the need to get a better understanding of what’s realistic in terms of setting goals and objectives. When we went into Afghanistan it’s fair to say that all of us – the international community, the Americans, the Afghans – did not fully understand the level of effort that would be needed to achieve some of the goals and objectives that we initially set for ourselves. 

I think if we could roll the clock back and go back in time, one of the lessons is that we might have tried to under promise more and then over deliver. When we went in in 2001 and 2002 we had talked about infrastructure that would be developed, how fast institutions would be built, how fast representative democracy might be able to take hold.

I think historians will look back and say they didn't really understand the complexities and the problems, they didn’t fully understand just how difficult this might all be.

Kevin Croke recently defended his PhD dissertation in the International Development program at Johns Hopkins-SAIS.  His dissertation, "Foreign Aid, Child Survival and Health System Strengthening in Tanzania and Uganda, 1996-2009," is based on fieldwork conducted in Tanzania and Uganda in 2009 and 2010.  He has previously worked with the World Bank, PEPFAR, Management Sciences for Health, Results for Development, and the East African NGO Twaweza.  He is currently working with the World Bank and Twaweza to develop a mobile phone survey platform for high frequency public service delivery monitoring in Tanzania.  At SAIS, Kevin has taught Introduction to Development and Comparative National Systems.

He has published articles in the American Political Science Review, American Journal of Political Science, Annual Review of Political Science, Journal of Conflict Resolution, Foreign Affairs, Foreign Policy, Journal of Democracy, World Policy Journal, and the SAIS Review.
