This is the third event in a three-part series on North Korea Human Rights hosted by APARC's Korea Program in the spring quarter.
Two experts in North Korea human rights issues, Minjung Kim of Save North Korea and Keith Luse of National Committee for North Korea, will discuss the range of humanitarian assistance to North Korea as well as challenges non-government organizations face from South Korea, the U.S. and North Korean governments in providing assistance.

Minjung Kim is Associate Executive Director at Save North Korea (SNK), a non-government organization focusing on human rights in North Korea; and a Vice President at Future Korea Media, a bi-weekly journal focusing on national security and politics in South Korea. Since joining SNK as a founding member 22 years ago, she has been managing various projects from producing mid-wave radio programs, bringing North Korean defectors into South Korea, helping them adjust socially and culturally, to sending leaflet-balloons and hidden cameras into North Korea. Kim is also a visiting researcher at Georgetown University and a research fellow at the Yonsei Institute for Modern Korean Studies. She is a Ph.D. candidate at Yonsei Graduate School of International Studies.

Keith Luse is Executive Director of the National Committee on North Korea (NCNK), an organization dedicated to promoting principled engagement between the United States and North Korea. NCNK members include representatives of US non-governmental organizations providing humanitarian assistance to North Korea. Other members are former US Ambassadors, nuclear scientists, and members of the academic community. NCNK members specialize in matters related to nuclear nonproliferation, Korean War POW/MIA/human remains; human rights and families divided by the Korean War, among other topics. Luse has made five visits to North Korea. The first occurring in 2002 on behalf of Senator Richard Lugar, then-Chairman of the Senate Agriculture Committee focused on monitoring and accountability regarding the distribution of U.S. food assistance. Subsequent trips for Senator Lugar were during his tenure as Chairman and later Ranking Member of the Senate Foreign Relations Committee.
Yong Suk Lee, deputy director of the Korea Program, will moderate the discussion.
Democracy promotion has been a longstanding goal of US foreign policy in the Middle East and elsewhere. President George W. Bush championed democracy promotion as a way to counter the ideology and extremism that led to the September 11, 2001 terror attacks against the United States. After Bush’s attempts ended in abject failure, President Barack Obama sought to repair relations with the Muslim world but also withdraw the US footprint in the Middle East. But Obama was forced to take a far more hands-on approach with the outbreak of the 2010-2011 uprisings known as the Arab Spring. President Donald Trump, who has displayed an almost allergic aversion to Obama’s policies, has openly embraced the region’s autocrats with little regard for their abuse of human rights or absence of attention to political or economic freedom. How the United States approaches the region matters – both for aspiring democrats and for those who wish to silence them. Despite the rise of Russia and China, the United States remains the sole superpower, with the loudest voice on the world stage. Thus, the shift from democracy promoter – albeit reluctantly at times – to authoritarian enabler has made the task of democratic political reform far more challenging for people across the Middle East. This discussion will examine the recent democracy promotion efforts of the United States, with a focus on the Obama and Trump years.

Sarah Yerkes is a fellow in the Carnegie Endowment for International Peace’s Middle East Program, where her research focuses on Tunisia’s political, economic, and security developments as well as state-society relations in the Middle East and North Africa. She has been a visiting fellow at the Brookings Institution and a Council on Foreign Relations international affairs fellow and has taught in the Security Studies Program at Georgetown University and at the Elliott School of International Affairs at the George Washington University. Yerkes is a former member of the State Department’s policy planning staff, where she focused on North Africa. Previously, she was a foreign affairs officer in the State’s Department’s Office of Israel and Palestinian affairs. Yerkes also served as a geopolitical research analyst for the U.S. military’s Joint Staff Strategic Plans and Policy Directorate (J5) at the Pentagon, advising the Joint Staff leadership on foreign policy and national security issues.
A U.S. foreign policy that cuts money to nongovernmental organizations performing or promoting abortions abroad has actually led to an increase in abortions, according to Stanford researchers who have conducted the most comprehensive academic study of the policy’s impact.
Eran Bendavid and Grant Miller — both associate professors at Stanford University School of Medicine and core faculty members at Stanford Health Policy — and doctoral candidate Nina Brooks find that abortions increased among women living in African countries where NGOs, such as the International Planned Parenthood Federation, were most vulnerable to the policy’s requirements.
The policy, widely known as the Mexico City Policy, explicitly prohibits U.S. foreign aid from flowing to any NGO that will not abide by the policy’s main condition: no performing or discussing abortion as a method of family planning, even if just in the form of education or counseling.
The policy has been a political hot potato since its inception. Enacted under Ronald Reagan in 1984, it’s been enforced by subsequent Republican administrations while Democrats in the White House revoked the policy within days of taking office.
The study by Brooks, Bendavid and Miller, published June 27 in The Lancet Global Health, looked at the policy’s effects in more than two dozen African countries over a span of 20 years under three presidents: Bill Clinton, George W. Bush and Barack Obama. It finds that, when the policy was in place during the Bush years, abortions were 40 percent higher relative to the Clinton and Obama administrations.
When the policy was suspended during Obama’s two terms, the research shows that the upward trend in abortion rates reversed.
A key reason for the uptick in abortions is that many NGOs affected by the policy also provide contraceptives – and funding cuts mean birth control is harder to get, said Brooks.
“By undercutting the ability to supply modern contraceptives, the unintended consequence is that abortion rates increase,” she said.
And the policy’s scope has expanded under the Trump administration. While it originally restricted aid directed only toward providing family planning and reproductive health services, President Trump has extended the policy to cover any group engaged in global health, including organizations providing services for HIV or child health – not just family planning.
Groundbreaking Research
The stakes are high. America is the world’s largest provider of development assistance and spent about $7 billion on international health aid in 2017. Many women in sub-Saharan Africa depend on this aid for contraceptives.
In sub-Saharan Africa, NGOs are often primary providers of family planning services. Two of the world’s largest family planning organizations – International Planned Parenthood Federation and Marie Stopes International – have forfeited large sums of U.S. cash for refusing to comply with the policy, according to news reports.
The research findings were based on records of nearly 750,000 women in 26 sub-Saharan African countries from 1995 to 2014. When the policy was in effect under George W. Bush, contraceptive use fell by 14 percent, pregnancies rose by 12 percent and abortions rose by 40 percent relative to the Clinton and subsequent Obama years – an impact sharply timed with the policy and in proportion to the importance of foreign assistance across sub-Saharan Africa.
The paper is the second study of the rule’s impact by Bendavid and Miller, who are both faculty members of Stanford Health Policy. The research is also one of the very few evidence-based analyses of the policy.
Their earlier research, the first quantitative, large-scale effort to examine the policy’s impacts, looked at a smaller set of African countries during the Clinton and Bush administrations and also found an increase in abortion rates when the policy was enacted in 2001.
“Our latest study strengthened our earlier findings because we were able to look at what happens when the rule was turned off, then on, and then off again,” said Bendavid, referring to the policy’s whipsawing under Clinton, Bush and then Obama.
Miller, who is the director of the King Center and a SIEPR senior fellow, says the team’s research reveals a deeply flawed policy.
“We set out to provide the best and most rigorous evidence on the consequences of this policy,” he said. “What we found is a clear-cut case of government action that everyone on all sides of the abortion debate should agree is not desirable.”
Signs of a Global Pushback
Brooks also notes that their findings may underestimate the rule’s full impact.
“The excess abortions performed due to the policy are more likely to be performed unsafely, potentially harming women beyond pregnancy terminations,” she said.
Under Trump, the international response to U.S. funding cuts has shifted. Norway, Canada and several other countries have pledged to increase funding of international NGOs affected by the policy – though not by enough to cover the expected shortfall, says Miller.
“This shows us,” he said, “that despite the intense partisanship in the U.S. over the rule and its implementation, there are ways that policymakers around the world can offset its effects – by ensuring higher levels of family planning funding, for example.”
Karl Eikenberry is a retired Army officer whose two tours of Afghanistan duty — and later service as ambassador to that nation — left him keenly aware of the limits of U.S. military power.
As a soldier, Eikenberry launched the still-ongoing effort to build an Afghan military force capable of fending off the Taliban. As a diplomat, he was stationed at the Kabul embassy during President Barack Obama’s surge that would eventually push American troop strength in Afghanistan to more than 100,000 service members in an attempt to improve security.
“Americans and the world have rightly been disappointed with the results of our costly military campaigns in Iraq and Afghanistan in the 21st century. Hugely expensive, protracted … and damaging to our country’s prestige abroad,” Eikenberry said Thursday to a Town Hall audience in Seattle.
Most studies that look at whether democracy improves global health rely on measurements of life expectancy at birth and infant mortality rates. Yet those measures disproportionately reflect progress on infectious diseases — such as malaria, diarrheal illnesses and pneumonia — which relies heavily on foreign aid.
A new study led by Stanford Health Policy's Tara Templin and the Council on Foreign Relations suggests that a better way to measure the role of democracy in public health is to examine the causes of adult mortality, such as noncommunicable diseases, HIV, cardiovascular disease and transportation injuries. Little international assistance targets these noncommunicable diseases.
When the researchers measured improvements in those particular areas of public health, the results proved dramatic.
“The results of this study suggest that elections and the health of the people are increasingly inseparable,” the authors wrote.
“Democratic institutions and processes, and particularly free and fair elections, can be an important catalyst for improving population health, with the largest health gains possible for cardiovascular and other noncommunicable diseases,” the authors wrote.
Templin said the study brings new data to the question of how governance and health inform global health policy debates, particularly as global health funding stagnates.
“As more cases of cardiovascular diseases, diabetes and cancers occur in low- and middle-income countries, there will be a need for greater health-care infrastructure and resources to provide chronic care that weren’t as critical in providing childhood vaccines or acute care,” Templin said.

Free and fair elections for better health
In 2016, the four mortality causes most ameliorated by democracy — cardiovascular disease, tuberculosis, transportation injuries and other noncommunicable diseases — were responsible for 25 percent of total death and disability in people younger than 70 in low- and middle-income countries. That same year, cardiovascular diseases accounted for 14 million deaths in those countries, 42 percent of which occurred in individuals younger than 70.
Over the past 20 years, the increase in democratic experience reduced mortality in these countries from cardiovascular disease, other noncommunicable diseases and tuberculosis between 8-10 percent, the authors wrote.
“Free and fair elections appear important for improving adult health and noncommunicable disease outcomes, most likely by increasing government accountability and responsiveness,” the study said.
What Templin and her co-authors found was democracy was associated with better noncommunicable disease outcomes. They hypothesize that democracies may give higher priority to health-care investments.

Foreign aid often misdirected
And yet, this connection between fair elections and global health is little understood.
“Democratic government has not been a driving force in global health,” the researchers wrote. “Many of the countries that have had the greatest improvements in life expectancy and child mortality over the past 15 years are electoral autocracies that achieved their health successes with the heavy contribution of foreign aid.”
They note that Ethiopia, Myanmar, Rwanda and Uganda all extended their life expectancy by 10 years or more between 1996 and 2016. The governments of these countries were elected, however, in multiparty elections designed so the opposition could only lose, making them among the least democratic nations in the world.
Yet these nations were among the top two-dozen recipients of foreign assistance for health.
Only 2 percent of the total development assistance for health in 2016 was devoted to noncommunicable diseases, which was the cause of 58 percent of the death and disability in low-income and middle-income countries that same year, the researchers found.
“Although many bilateral aid agencies emphasize the importance of democratic governance in their policy statements,” the authors wrote, “most studies of development assistance have found no correlation between foreign aid and democratic governance and, in some instance, a negative correlation.”
Autocracies such as Cuba and China, known for providing good health care at low cost, have not always been as successful when their populations’ health needs shifted to treating and preventing noncommunicable diseases. A 2017 assessment, for example, found that true life expectancy in China was lower than its expected life expectancy at birth from 1980 to 2000 and has only improved over the past decade with increased government health spending. In Cuba, the degree to which its observed life expectancy has exceeded expectations has decreased, from four-to-seven years higher than expected in 1970 to three-to-five years higher than expected in 2016.
“There is good reason to believe that the role that democracy plays in child health and infectious diseases may not be generalizable to the diseases that disproportionately affect adults,” Bollyky said. Cardiovascular diseases, cancers and other noncommunicable diseases, according to Bollyky, are largely chronic, costlier to treat than most infectious diseases, and require more health care infrastructure and skilled medical personnel.
The researchers hypothesize that democracy improves population health because:
When enforced through regular, free and fair elections, democracies should have a greater incentive than autocracies to provide health-promoting resources and services to a larger proportion of the population;
Democracies are more open to feedback from a broader range of interest groups, more protective of media freedom and might be more willing to use that feedback to improve their public health programs;
Autocracies reduce political competition and access to information, which might deter constituent feedback and responsive governance.
Various studies have concluded that democratic rule is better for population health, but almost all of them have focused on infant and child mortality or life expectancy at birth.
Over the past 20 years, the average country’s increase in democracy reduced mortality from cardiovascular disease by roughly 10 percent, the authors wrote. They estimate that more than 16 million cardiovascular deaths may have been averted due to an increase in democracy globally from 1995 to 2015. They also found improvements in other health burdens in the countries where democracy has taken hold: an 8.9 percent reduction in deaths from tuberculosis, a 9.5 percent drop in deaths from transportation injuries and a 9.1 percent mortality reduction in other noncommunicable disease, such as congenital heart disease and congenital birth defects.
“This study suggests that democratic governance and its promotion, along with other government accountability measures, might further enhance efforts to improve population health,” the study said. “Pretending otherwise is akin to believing that the solution to a nation’s crumbling roads and infrastructure is just a technical schematic and cheaper materials.”
The other researchers who contributed to the study are Matthew Cohen, Diana Schoder, Joseph Dieleman and Simon Wigley, from CFR, the University of Washington-Seattle and Bilkent University in Turkey, respectively.
Funding for the research came from Bloomberg Philanthropies and the Bill & Melinda Gates Foundation. Stanford’s Department of Health Research and Policy also supported the work.
A dramatic opening created by the unique strategic outlooks and personalities of Moon Jae-in, Kim Jong Un and Donald Trump instigated a series of highly symbolic summits in the early months of 2018. The process kicked off by those summits has bogged down, however, as the necessary compromises for an agreement between the United States and North Korea have proved elusive. This year's Koret Workshop will therefore invite experts from a variety of areas in order to reflect on what the stumbling blocks have been as well as prospects for overcoming them. Conference participants will work towards better understanding and supporting potential emerging solutions to the persistent conflict on the Korean Peninsula.

Scholars have credited a model of state-led capitalism called the developmental state with producing the first wave of the East Asian economic miracle. Using historical evidence based on original archival research, this talk offers a geopolitical explanation for the origins of the developmental state. In contrast to previous studies that have emphasized colonial legacies or domestic political factors, I argue that the developmental state was the legacy of the rivalry between the United States and Communist China during the Cold War. Responding to the acute tensions in Northeast Asia in the early postwar years, the United States supported emergency economic controls in Japan, South Korea, and Taiwan to enforce political stability. In response to the belief that the Communist threat would persist over the long term, the U.S. strengthened its clients by laying the foundations of a capitalist, export-oriented economy under bureaucratic guidance. The result of these interventions was a distinctive model of state-directed capitalism that scholars would later characterize as a developmental state.
I verify this claim by examining the rivalry between the United States and the Chinese Communists and demonstrating that American threat perceptions caused the U.S. to promote unorthodox economic policies among its clients in Northeast Asia. In particular, I examine U.S. relations with the Chinese Nationalists on Taiwan, where American efforts to create a bulwark against Communism led to the creation of an elite economic bureaucracy for administering U.S. economic aid. In contrast, the United States decided not to create a developmental state in the Philippines because the Philippine state was not threatened by the Chinese Communists. Instead, the Philippines faced a domestic insurgency that was weaker and comparatively short-lived. As a result, the U.S. pursued a limited goal of maintaining economic stability instead of promoting rapid industrialization. These findings shed new light on the legacy of statism in American foreign economic policy and highlight the importance of geopolitics in international development.

The Freeman Spogli Institute for International Studies (FSI) at Stanford University is pleased to announce that former U.S. Ambassador and World Food Programme (WFP) Director Ertharin Cousin will serve as this year’s Frank E. and Arthur W. Payne Distinguished Lecturer and Visiting Fellow at the Center on Food Security and the Environment (FSE).
Cousin brings over 25 years of experience addressing hunger and food security strategies on both a national and international scale. As U.S. Ambassador to the United Nations Agencies for Food and Agriculture, she focused on advocating for longer-term solutions to food insecurity and hunger, and at WFP she addressed the challenges of food insecurity in conflict situations.
“Dr. Cousin’s outstanding leadership at the WFP and extensive experience in public service exemplifies the attributes we seek for Payne Lecturers,” says FSI Director Michael McFaul. The Payne Distinguished Lectureship is awarded to scholars with international reputations as leaders, with an emphasis on visionary thinking, practical problem solving, and the capacity to clearly articulate an important perspective on the global political and social situation. Past Payne Lecturers include Bill Gates, Nobel Laureate Mohamed El Baradei, UNAIDS Executive Director Peter Piot, and novelist Ian McEwan.
As a visiting fellow with FSE, Cousin will be working to further her research focus on global food security and humanitarian efforts. In November 2015, FSE welcomed Cousin as the featured speaker in their Food and Nutrition Symposium series, where she presented her paper “Achieving food security and nutrition for the furthest behind in an era of conflict and climate change.” FSE Director, Roz Naylor, sees Cousin’s appointment as a pivotal opportunity for FSE and FSI to advance a global agenda on food security and human rights. “Ertharin Cousin is one of the most inspirational leaders we could ever hope to attract to Stanford as a year-long visitor,” Naylor says.
“This is a truly humbling, yet exciting prospect,” says Cousin. “This position provides an opportunity for scholarly work and dialogue with distinguished academics across Stanford's schools and policy institutes. I also look forward to the opportunity to convene thought leaders from a broad variety of backgrounds, who can help us explore some of the intractable issues plaguing humanitarian and development practitioners today.”
Following the completion of her term with the WFP, Cousin accepted an appointment as a Distinguished Fellow with The Chicago Council on Global Affairs, which conducts research on food and agriculture, global cities, economics, energy, immigration, security, public opinion, and water. Cousin hopes her appointments can provide a unique collaborative opportunity to expand her work on food security and nutrition issues.
“In my career I have never before been given the opportunity of pursuing intellectual inspiration. Just thinking about the ‘what’s possible’ gives me genuine pleasure,” Cousin said.

Abstract: Both the academic and policy making worlds have been dominated by three explanations for development, understood broadly as democratization and rising levels of per capita income. The first argument is modernization theory which assumes that if polities are provided with adequate resources, especially investment, they will develop. The second argument is institutional capacity approaches which focus on the ability of the state to maintain order. The third argument is rational choice institutionalism which sees deveopment as a rare event resulting from the self interested calculations of elites. Happenstance and path-dependence play major roles for rational choice instititoinalism. All three of these approaches suffer from major gaps. All three, however, are consistent with the view that external state-building efforts will only be successful if the objectives of external and internal elites are complmentary. This suggests that for most polities the best that external actors can accomplish is Good Enough Governance: security, some service provision, some economic growth.

The existence of two Chinese states—one controlling mainland China, the other controlling the island of Taiwan—is often understood as a seemingly inevitable outcome of the Chinese civil war. Defeated by Mao Zedong, Chiang Kai-shek’s Nationalists fled to Taiwan to establish a rival state, thereby creating the “Two Chinas” dilemma that vexes international diplomacy to this day. Accidental State challenges this conventional narrative to offer a new perspective on the founding of modern Taiwan.
Hsiao-ting Lin marshals extensive research in recently declassified archives to show that the creation of a Taiwanese state in the early 1950s owed more to serendipity than careful geostrategic planning. It was the cumulative outcome of ad hoc half-measures and imperfect compromises, particularly when it came to the Nationalists’ often contentious relationship with the United States.
Taiwan’s political status was fraught from the start. The island had been formally ceded to Japan after the First Sino–Japanese War, and during World War II the Allies promised Chiang that Taiwan would revert to Chinese rule after Japan’s defeat. But as the Chinese civil war turned against the Nationalists, U.S. policymakers reassessed the wisdom of backing Chiang. The idea of placing Taiwan under United Nations trusteeship gained traction. Cold War realities, and the fear of Taiwan falling into Communist hands, led Washington to recalibrate U.S. policy. Yet American support of a Taiwan-based Republic of China remained ambivalent, and Taiwan had to eke out a place for itself in international affairs as a de facto, if not fully sovereign, state.
Biography
Hsiao-ting Lin is a research fellow and curator of the East Asia Collection at the Hoover Institution. He holds a BA in political science from National Taiwan University (1994) and an MA in international law and diplomacy from National Chengchi University in Taiwan (1997). He received his DPhil in oriental studies in 2003 from the University of Oxford, where he also held an appointment as tutorial fellow in modern Chinese history. In 2003–4, Lin was a postdoctoral fellow at the Institute of East Asian Studies, University of California at Berkeley. In 2004, he was awarded the Kiriyama Distinguished Fellowship by the Center for the Pacific Rim, University of San Francisco. In 2005–7, he was a visiting fellow at the Hoover Institution, where he participated in Hoover’s Modern China Archives and Special Collections project. In April 2008, Lin was elected a fellow of the Royal Asiatic Society of Great Britain and Ireland for his contributions to the studies of modern China’s history.
Lin’s academic interests include ethnopolitics and minority issues in greater China, border strategies and defenses in modern China, political institutions and the bureaucratic system of the Chinese Nationalist Party (Kuomintang), and US-Taiwan military and political relations during the Cold War. He has published extensively on modern Chinese and Taiwanese politics, history, and ethnic minorities, including Accidental State: Chiang Kai-shek, the United States, and the Making of Taiwan (Harvard University Press, 2016); Modern China’s Ethnic Frontiers: A Journey to the West (Routledge, 2011); Breaking with the Past: The Kuomintang Central Reform Committee on Taiwan, 1950–52 (Hoover Press, 2007); Tibet and Nationalist China’s Frontier: Intrigues and Ethnopolitics, 1928–49 (UBC Press, 2006), nominated as the best study in the humanities at the 2007 International Convention of Asia Scholars; and over a hundred journal articles, book chapters, edited volumes, reviews, opinion pieces, and translations. He is currently at work on a manuscript that reevaluates Taiwan’s relations with China and the United States during the presidency of Harry Truman to that of Jimmy Carter.
